Welcome to our Patient Portal, providing you with convenient, secure access to Our Offices from the comfort and privacy of your home.

The Portal is for non-urgent care only. If you have a medical emergency, call 911.

Through the Portal, you'll be able to:

• Communicate with practice, securely and efficiently.


• Review your statements.


• View date and time of upcoming appointments.


• Request a prescription refill from pre-populated list of currently refillable prescriptions.


• View your Personal Health Record.


• Manage your personal information.

Your e-mail address will be required for Portal access, but rest assured it will be held in a secure, safe environment and will only be used for matters pertaining to your medical care. Once Our Offices has your email address, you will be web enabled and will receive an email from us containing your username and password to log into the Patient Portal. PLEASE MAKE SURE THAT AFTER YOU LOG IN--YOU CLICK ON THE BLUE PHR TAB ON THE BOTTOM OF THE SCREEN in order to view your Patient Health Record. Also if you are a new patient to the practice, be sure to fill out all the questionnaires under the intake tab (Review of Systems, Family History, Past Medical History, Allergies, Past Surgical History)
